An NDIS audit checks that your organisation meets the NDIS Practice Standards and Quality Indicators that apply to your registration. The best preparation is not a last-minute rush. It is keeping your documents, records and evidence current all year. This checklist covers what auditors commonly look for.
1. Know which audit you need
Providers of lower-risk supports usually need a verification audit, which is mostly a desktop review of documents. Providers of higher-risk or more complex supports usually need a certification audit, which includes interviews, site visits and review of participant files. Your registration groups decide which applies.
2. Policies and procedures
- Every policy your registration requires is in place and approved
- Each policy has a version number, approval date and next review date
- Staff have read and acknowledged current versions
- Policies match what your team actually does day to day
3. Registers
- Incident register, including reportable incidents and outcomes
- Complaints and feedback register, with resolution and learning
- Risk register, reviewed regularly
- Continuous improvement register
- Training and worker screening registers
- Conflict of interest and legislative compliance registers
4. Workers
- Current NDIS worker screening clearances for workers in risk-assessed roles
- Working With Children Checks where required
- Qualifications, first aid and mandatory training are current
- Inductions and NDIS Code of Conduct acknowledgements on file
5. Participant records
- Service agreements signed and current
- Consent records and support plans up to date
- Risk assessments completed and reviewed
- Progress notes that show supports were delivered as agreed
- Evidence of participant feedback and choice and control
6. Incidents and complaints
Auditors look for a working system, not a perfect record. Show how incidents and complaints are recorded, investigated, resolved and used to improve. Reportable incidents must be notified to the NDIS Commission within the required timeframes.
7. Evidence pack
Map your evidence to each applicable Practice Standard so the auditor can find it quickly. A clear, organised evidence pack saves time on the day and shows good governance.
